6. Signing of the Memorandum of Agreement (MOA) or "Kasunduan" and issuance of membership ID
7. Processing of Connection Order
8. Installation of Meter

2. Photo of the establishment/house (front view) with SERVICE ENTRANCE
3. Electrical lay-out plan signed by PEE (Original Copy)
4. Electrical permit (Original Copy)
5. Certificate of final electrical inspection signed by the Municipal Engineer/ BFP/ PEE (Original Copy)
6. Fire Safety Clearance (Electrical Clearance) from BFP (Original Copy
7. 2 Pcs. 2x2 picture
8. 1 Valid ID (Photocopy)
9. Electric bill from nearest neighbor (Photocopy)
10. Proof of Lot Ownership / Affidavit of Ownership
11. Notarized Letter of Undertaking (If Applicable)
12. Notarized Special Power of Attorney (If Applicable)
