6. Signing of the Memorandum of Agreement (MOA) or "Kasunduan" and issuance of membership ID
7. Processing of Connection Order
8. Installation of Meter

2. Electrical Lay-out Plan with riser diagram and computation of loads (Original)
3. Electrical Permit and Certificate of Final Electrical Inspection (Original)
4. Fire Safety Inspection Certificate (Original)
5. 2 Pcs. 2x2 picture
6. 1 photocopy of Valid ID/CTC or Sedula
7. Photocopy of electric bill of nearest neighbor
8. Proof of Lot Ownership / Affidavit of Ownership
9. Notarized Letter of Undertaking (If Applicable)
10. Notarized Special Power of Attorney (If Applicable)
